Do I really need a crown? When a filling is no longer enough.

Sometimes saving a tooth means knowing when not to simply fill it.

“Can’t you just put a filling on it?”

It’s a question dentists hear all the time—and it’s a very reasonable one.

When part of a tooth is damaged, choosing the smallest possible restoration may seem like the most conservative option. But there comes a point when a filling is no longer enough to protect what remains of the tooth.

In those cases, a crown isn’t necessarily about doing more dentistry. Sometimes, it’s about preventing a much bigger problem later.

A filling repairs. A crown protects.

A filling replaces a relatively small area of tooth structure lost to decay, wear, or damage. The remaining natural tooth provides most of the strength.

But as more tooth structure is lost, the situation changes.

A large filling can replace what is missing, but it cannot always reinforce weakened walls of natural tooth. Under everyday chewing forces, those remaining walls may flex or fracture.

A crown works differently. It covers and protects the compromised tooth, helping distribute biting forces across a larger surface.

So, when might a filling no longer be enough?

1. A large portion of the tooth is already missing. When there isn't enough healthy tooth structure surrounding a filling, simply making the filling larger may leave the tooth vulnerable to fracture.

2. The tooth already has a very large filling. Replacing an old filling with an even larger one isn't always the safest long-term solution.

3. The tooth has cracks or weakened cusps.

4. The tooth has undergone root canal treatment. Especially in posterior teeth, significant loss of tooth structure may mean the tooth needs additional protection after root canal treatment.

5. The tooth is exposed to significant biting forces. Molars, bruxism, clenching, existing wear and the patient's bite all influence the decision.

The biggest filling possible isn't always the most conservative treatment.

Conservative dentistry isn't simply about choosing the smallest restoration today. It's about preserving as much healthy tooth structure as possible for as long as possible. Sometimes that means a small filling. Sometimes it means an inlay, onlay, or partial-coverage restoration. And sometimes a crown provides the protection the tooth needs. The right choice depends on how much healthy tooth remains—not simply on the size of the cavity.

Before saying yes to a crown, ask to see why.

Ask your dentist to show you the X-ray, photographs, existing restorations, cracks, or areas of weakened tooth structure that are influencing the recommendation.

At Prime Dental, we believe treatment decisions should begin with diagnosis—not with a procedure.

The goal isn't to place a crown whenever possible. The goal is to preserve your natural tooth whenever possible—and protect it when necessary.
